Below you’ll find information about how to file a complaint with relevant authorities, contact licensing boards, and request your records.
This information is provided in accordance with Texas House Bill 4224 and Texas Health & Safety Code §181.105.

With your Clinician:
f you're unhappy with any part of your experience, we encourage you to share your feedback directly with your clinician. Hearing about your experience helps us understand what went wrong, make things right, and refine our approach to help prevent similar issues for future patients. You're welcome to submit your concerns through the Spruce app, or if you'd prefer a more personal conversation, we can schedule a phone or video call.
With the Executive Council:
The Texas Behavioral Health Executive Council investigates and prosecutes professional misconduct committed by the following professionals: psychologists, psychological associates, professional counselors, marriage and family therapists, social workers, and licensed specialists in school psychology. If you would like to report suspected professional misconduct, you can contact the Texas Behavioral Health Executive Council by phone at 1-800-821-3205.With the Attorney General:
The Attorney General of Texas collects and maintains records of consumer complaints to protect Texas residents from unlawful business practices. Information on how to submit a consumer complaint may be found here on the Attorney General's website.Please be aware that, under Texas law, consumer complaints are open to the public. Therefore, the Attorney General's office requests that you do not include sensitive or confidential information in your complaint.

You have the right to access and receive copies of your health care records. To ensure the security of your confidential Protected Health Information (PHI), PIPS requires that all record requests be submitted through our secure HIPAA-compliant portal.
Instructions: Log in to your Spruce Account. Send a message to Dr. Bartholomew stating a request for a Release of Information document. One will be sent to your patient portal within 48 business hours. Complete the form and return it via the portal.
Timeline: In accordance with Texas law, we will respond to your request for records within 15 business days.
Fees: PIPS will charge a reasonable, cost-based fee for the retrieval and copying of records as permitted by Texas Law.
